1. Written evidence of required immunizations must be presented to the school within 30 school days of admission. The Department of Health and Social Services Student Immunization Record Form should be used.
2. The student' parent(s)/guardian will be notified of noncompliance after 25 days.
3. The district attorney of the county in which the student resides will be notified of noncompliance after 60 days. Court action will subsequently be taken.
