MS Athletic Forms

Students must have a current WIAA physical card (good for two years) or 
an alternate year physical card (opposite a physical year), athletic code book, and $40 user fee for each sport.

Middle School Athletic Code
Parent and Athlete signature is required on last page.
WIAA Physical Form
A physical is required for sports every two years. Physician and parent signature required.
Alternate Year Physical Form
Complete this form in alternate years when a physical is not needed. Parent signature required.
