The following athletic offerings are available at the middle school level:

Paperwork and fees needed:
1. Current physical card (good for two years-physican's signature)
or alternate year card (every other year-parent signature)
2. Athletic code (one form per year)
3. Concussion form (one form per year)
4. Athletic fee ($40 per sport)
5. Other paperwork required by individual coach

NOTE: River Valley Middle School sports are funded using dollars from Fund 80 (Community Service Fund) and are, therefore, open for participation of all middle school students in the community, including parochial students and home school students.
